    Complete the online form sent by Luna Health before your visit, and also fill out any required school, sports league, or camp forms so the clinician can finalize everything at the appointment.

    Complete appointment and get paperwork

    At the visit, we complete the sports physical exam, review your forms for accuracy, and send you home with the completed paperwork you need.

All forms must be completed prior to your appointment or you may have to come back another time.
